What this form is.

  • A week-by-week record of your supervised experience hours as an associate.
  • Column A is direct clinical hours. Column B is non-clinical hours, split into B1 individual or triadic supervision and B2 group supervision. Column C is the week's total.
  • Two caps apply: no more than 40 hours in a week may be counted, and no more than 6 hours of supervision in a week.

Who signs it

You and your supervisor, week by week. California's 2022 supervision regulation changes are the source for the weekly signature expectation.

Common mistakes.

Most are easy to catch while the work is fresh.

  • Saving it all up and asking for one signature at the end. The log is weekly for a reason.
  • Putting supervision hours in column A. Supervision is column B.
  • Recording a week over the caps and assuming the board will simply count the extra. It may not.
  • Weeks with a gap in the middle, because a supervisor was away and nobody logged it.
